PCMH 2019 Certificate


The Patient-Centered Medical Home (PCMH) is a care delivery model whereby patient treatment is coordinated by the primary care physician to ensure patients receive the necessary care when and where they need it, in a manner that is easily understandable.

The objective is to have a centralized setting that facilitates partnerships between individual patients and the patient’s family and their personal physicians. Care is facilitated by health information exchange sources and other means to assure that patients get the indicated care when and where they need and want it in an appropriate manner.

Lowcountry Pediatrics has been recognized as a Patient Centered Medical Home by The National Committee for Quality Assurance for achievement of recognition for systematic use of patient-centered, coordinated care management processes.
